Dean Turner Training 1-on-1 Online Coaching Questionnaire

Upon completing this questionnaire, you will be contacted via email to set up an introductory call. On the introductory call, we will discuss your training history, physical limitations, goals, etc. in a bit more detail. From there, you will need to determine if the partnership is worth pursuing.

Assuming you believe the partnership is worth pursuing, you will need to choose between the following plan options and make your commitment:

1 Month = $500 (Payable via (1) Venmo or (2) CashApp or (3) PayPal)

3 Months = $1,200 Total or $400 per month (Payable via (1) Venmo or (2) CashApp or (3) PayPal or (4) Payment Link using Debit or Credit Card)

6 Months = $1,800 Total or $300 per month (Payable via (1) Venmo or (2) CashApp or (3) PayPal or (4) Payment Link using Debit or Credit Card)

Your payment will act as your form of commitment.

***PLEASE NOTE: NO WORK WILL BE DONE ON CREATING YOUR CUSTOMIZED TRANSFORMATION PLAN UNTIL YOUR PAYMENT IS SUBMITTED***

Once your payment is submitted, we are officially a coach/client pair. From there, we will begin working together in an intimate manner to (hopefully) help you achieve your goal(s). The business relationship will continue (at least) until you've reached the end of the plan term. At that time, you will need to choose whether to re-up (by purchasing another plan) or part ways.
