After School Snack Site Agreement
The site Sponsor agrees to:
* Serve a minimum of 15 snacks per day.
* Serve snacks only to children between the ages of 1 and 18 years of age who are actively enrolled in the program.
*Ensure that no snacks are removed from the site.
*Provide adequate supervision and accurate point of service during snacks.
*Accurately maintain the district approved roster daily.
*Submit all reports and records to the school Nutrition Manager monthly.
*Report to the school nutrition manager any changes in the number of snacks required as attendance fluctuates.
* Provide children with regularly scheduled activities in an organized, structured and
supervised environment; and
* include educational or enrichment activities.
This is to certify that I have read and agree to the above statements and understand that any violation of these guidelines may result in prosecution under applicable State and Federal Statues. In addition, sites with program violations will not be considered for renewal in subsequent years.
