I understand that the Memory Café is a shared community space where the health, safety, and well-being of all participants, caregivers, and volunteers is a priority.
I will not attend if I or someone in my household has tested positive for COVID-19 or any contagious illness in the past 5 days, or is currently experiencing symptoms such as fever, cough, or shortness of breath.
I agree to follow any posted guidelines regarding masks, hygiene, or distancing as determined by the Memory Café organizers in accordance with local health recommendations.
I understand that the Memory Café is not a medical or emergency care facility, and that participants with memory challenges must be accompanied by a responsible caregiver if needed for their support.
