This form has three sections.  You are required to answer questions in all three sections in order to submit a calendar request.

IMPORTANT NOTICE:  No two speech tournaments may be scheduled on the same date in sites closer than sixty miles of each other without the approval of the Chief Operating Officer and the coaches involved. This rule applies to both speech and debate tournaments (IHSFA Tournament Manual, X.A).
