Pre-Approved Absence Form
This form is to be used to request absences for students that fall outside of the excused or exempt absences listed in the handbook.

Students may be excused from school for the following reasons:

  1. Personal illness (extended illnesses may require a doctor’s statement)

  2. Critical illness or death of a family member

  3. Funeral

  4. Military Connected Families (absences related to deployment or return)

Exempt Absences

Under certain circumstances, the law requires the school to authorize the following absences as excused:

  1. serving as a page or honoree of the General Assembly (IC 20-33-2-14 ) 

  2.  when subpoenaed to testify in court (IC 20-33-2-16)

  3.  the student or a member of the student’s household exhibits or participates in the Indiana State Fair for educational purposes (IC 20-33-2-17.7).

Each student is allotted 3 per year before absences are then considered unexcused. This form must be filled out 5 schools days prior to the planned absence.
