Fossil Rim 5th Grade Overnight Field Trip

Your child is eligible to participate in a school-sponsored activity requiring transportation to a location away from the school building and grounds. This activity will take place under the guidance and supervision of employees from St. Mary’s Catholic School. A brief description of the activity follows:

Name of Event: Overnight 5th Grade field trip to Fossil Rim Wild Life Center
Designated Supervisor of Activity: Ms. Cruz
Date(s): April 4-5, 2019. Departing on the 4th at 7:30am, returning on the 5th around 10-11am.
Method of Transportation: (Parent drivers) If you are interested in driving and or chaperoning please send Ms. Cruz an email and if you are selected Ms. Cruz will be in contact with additional information. For those who are asked to drive the office will need a copy of your driver's license and insurance information, thank you.
Student Requirements: Information will be coming in an email from the office
Cost: 95.00 per student and chaperone
Due by: March 29, 2019

****ADDITIONAL INFORMATION will be sent in a separate email once we have your child's permission slip, this email will include: behavior contract that must be signed before leaving for the trip, packing list and itinerary.****

In consideration for the making of the arrangements for this trip, I/We hereby release and save harmless the St. Mary’s Catholic School, all its employees and the Diocese of Austin from any and all liability arising to my child as a result of this trip. I also agree to allow my child to travel on this trip in a private vehicle.

If emergency treatment is required for my child, I/We authorize St. Mary’s Catholic School personnel to follow area EMS regulations in caring for my child.
