Important Information:

Families from any church school in the River Region may register; however, acceptance is at the discretion of LIFT. This form contains several sections so please read carefully and provide accurate information. You will be required to complete questions with an asterisk.

The Registration (LIFT) & Enrollment (Class) Process:

LIFT REGISTRATION (March 10th):
Beginning on March 10th, complete this online registration form. Complete the Disclaimer & Liability Release Form and the Medical Release Form found on the website. Copy the front and back of your insurance card on the bottom of the Medical Release Form. Mail these documents to Sharon Hubbard at P. O. Box 241738, Montgomery, AL, 36124, and pay all LIFT registration fees.

CLASS ENROLLMENT :
Enrollment codes will be sent once student registration is approved. Codes will be valid for 48 hours. You will NOT receive a code until ALL registration fees and paperwork have been received. Complete your online enrollment and pay a $20 deposit for each class by mail. Note: some classes require copy or student notes fees with your deposit. This information is found on the enrollment form.

FEES & TUITION:
Registration Fee: CHECKS SHOULD BE MADE PAYABLE TO LAKEVIEW INSTITUTE FOR TEENS
Returning Families - $250.00 per family for the first student; $150.00 per family for each additional student.
New Families - $275 per family for the first student; $175 per family for each additional student.

Clothing Fee: (Students are required to wear LIFT t-shirts on each campus. See the clothing policy for more details.)
Short Sleeve T-Shirts $15
Long Sleeve T-Shirts $20
Hoodies $30

Tuition:
Each class requires a $20 deposit upon enrollment.
Some classes may also require that copy or notebook fees be paid with the class deposit. Please refer to the enrollment form or contact the instructor for more information. Class tuition can be found in the class descriptions and is paid directly to the instructor each quarter. Class deposits, copy or notebook fees, and class tuition are paid directly to the instructor. Payment information is listed on the class enrollment form and in the class descriptions. Please contact the instructor for additional information.

Registration Payments:
We prefer that you pay all LIFT registration fees via check or money order. PLEASE NOTE THAT LIFT FEES ARE NON-REFUNDABLE. CHECKS SHOULD BE MADE PAYABLE TO LAKEVIEW INSTITUTE FOR TEENS
