Long Term Transportation Request

Children may be transported to and/or from an address other than home (e.g. day care provider) if the parent or guardian completes a Long Term Transportation Request.

All forms must be submitted by June 1st for the following school year.

Requests must be made on a yearly basis and approval/denial is based on current district routes and transportation numbers.

We are only able to provide transport 5 days a week to one location. The location must be a district address and meet the requirements outline on the Transportation website below.

This request is made with the full understanding that it must meet all guidelines set by the Transportation Department. This request must be approved by the Transportation Supervisor and the school principal. The guidelines are found on the ESM website. *

I understand that I will be contacted directly by the Transportation Department once a final decision has been made.
