By registering my student, I acknowledge and accept the following:

Student Responsibilities:

My student will use their bachi (sets will be provided to students who need a set).
My student may not loiter at Davis Middle School before or after practice.
My student will attend the practice sessions assigned to them for their level of performance.

My student is bound by the Dublin City Schools Code of Conduct.

Parent Responsibilities:

Parents are responsible for picking up their children from practice sessions or performance returns no later than 15 minutes after conclusion of practice or arrival at Davis Middle School. Repeated tardiness regarding student pickup will result in a student's inability to participate in the program. We understand emergencies arise.

Dublin Taiko Group - Participation Fee

The Dublin Taiko participation fee for 2023-2024 is $50 per student. If you are unable to afford the participation fee, please indicate below.

Dublin Taiko does not receive financial support from Dublin City Schools. All operating expenses are covered through Booster fundraising and the participation fees.

Your student’s participation fee will cover a portion of your student's annual expenses:

❏ Use and maintenance of uniform
❏ Tabi (footwear)
❏ Dublin Taiko uniform t-shirt (issued to beginners only)
