Field Trip Request Form
Field trips are instructional related outings that specifically support or extend a planned learning activity/activities.  To be reimbursed for any admission related expenses, this form must be completed and reviewed with your Consulting Teacher PRIOR TO TAKING THE TRIP.    

Gateway will reimburse one-time admission related expenses for any approved field trips up to $150.00 per calendar year for each student.  Reimbursement requests may also be submitted to cover admission costs for one adult per trip.  

A copy of the completed  Field Trip form, with proper receipts for admission expenses, must be included with your monthly Reimbursement requests.

This form must be used to submit requests for annual passes to educational venues like museums, zoos, the Exploratorium, etc. .  Given the various pricing structures, you need to work directly with your Consulting Teacher in completing this request prior to purchasing any passes. If approved these expenses will approximate the annual cost for one student and one adult.  

Note: This form does not apply to school sponsored field trips like performances at the Gallo, or sponsored trips to the W.O.W. museum, etc.  Admission expenses for the student and one adult for school sponsored trips will not count towards the $150.00 annual field trip maximum and, per past practice, will be automatically deducted from your stipend account.
