Ray Templeton MHS Boosters Athletic Scholarship TEACHER, COUNSELOR OR ADMIN Recommendation Form
The Student Athlete above is applying to receive the Ray Templeton Athletic Scholarship from the Montgomery High School Booster Club. Please complete the following form as a recommendation for this student athlete. Simply submit the google form, or if filling out a hard copy, please submit in a sealed envelope with your signature across the seal and return to the Booster mailbox located in the Montgomery High School office. The recipient may or may not be the teams’ best athlete, but has shown exceptional enthusiasm, participation, effort, spirit and dedication to warrant to this special recognition.

Notice: THIS APPLICATION MUST BE IN THE MAIN OFFICE NO LATER THAN May 12, 2024 TO PREVENT APPLICANT’S DISQUALIFICATION.
