To withdraw from any tournament that you have registered for, please contact the PPGA Tournament Department at PPGATournaments@PGAHQ.com, (267) 622-6170 or complete the following form at least 7 days prior to the tournament date by 5 p.m.  All withdrawals after the withdrawal deadline will forfeit their tournament entry fee.
