780 Fifth Avenue, LLC, shall provide a mailing address, mail service, directory signage in first floor lobby, telephone forwarding, and conference room availability in exchange for $500.00 per month, plus sales tax paid by Client to the City of Naples, County of Collier, and/or State of Florida. If Client should make use of other services (i.e. fax, photocopy, postage, UPS, Federal Express, etc.), provided by 780 on Fifth, such charges for those services will be billed separately.

A deposit of $500.00 shall be held by 780 Fifth Avenue, LLC, during the service period. The deposit is refundable provided that all payments are current.

 A one-time non-refundable set up fee of $225 is required with this agreement.

All incoming mail shall be sorted and held for Client's pickup at hours designated by 780 Fifth Avenue, LLC. All incoming telephone calls will be forwarded to a telephone number of Client's choosing.

780 on Fifth shall provide Client twelve (12) hours of conference room time per month between 9:00am and 5:00pm Monday through Friday (excluding holidays) based on conference room availability.  The monthly allotment of hours is non-cumulative from month to month.  Additional conference room hours can be purchased on an hourly basis at fifty dollars ($50.00) per hour for the large conference room and forty dollars ($40.00) for the small conference room.

All payments shall be made to 780 Fifth Avenue, LLC, on or before the first of each calendar month. Should payment not be received by the third (3rd) of each month, a late charge of fifty dollars ($50.00) will be assessed, and service may be terminated.

780 Fifth Avenue, LLC, or 780 on Fifth, shall not be liable for any loss of business or damages of any sort occurring in connection with, or incidental to the furnishing of, or failure to furnish, mail or telephone service.

This agreement can be terminated by thirty (30) days written notice by either party.
