Do you have preexisting medical conditions that we should be aware of?
Knowing this information can help us better react to medical emergencies that may take place. If you have preexisting conditions that require a particular course of treatment should symptoms arise, we would also welcome advice about how to react, although ask that you recognize medical responsibility for your welfare ultimately does not then fall on us or our volunteers. As well, if there is an emergency contact you would like to share, please do include this here as well.
**Note on Emergency Contact Info**
We have sometimes asked for emergency contact info, and some organizers do really like it: The big thing  is to ensure you are using it if you do ask the question, i.e. making sure that it is shared with captains if needed or is available on your person should this info be needed. For the most part, we find that injuries in our recreational leagues are rarely serious enough to make one unable to share emergency info when they are hurt, and also that health card info/ ID's often have emergency contacts can be used when it is called for. Bottom line, please only ask the question if you'll take steps to use the answer!
