If you would like to attend the upcoming legal name change clinic provided by Southern Legal Counsel's Transgender Rights Initiative on
Tuesday, October 21st, from 6:30pm-8:30pm, please complete the registration form below. This clinic is a partnership between SLC's Trans Rights Initiative and incredible pro bono attorneys from the law firms of Leòn Cosgrove Jimenez, LLP and Greenberg Traurig, P.A.
You
will not receive a confirmation email after you register, so please don't worry -
as long as you complete and submit the registration form below, you will be registered for the clinic. Prior to the date of the clinic,
you will receive a
packet of forms that are specific to the county where you reside (and specific to whether you are seeking a name change for an adult or a minor), and you will also receive a Zoom link to attend the clinic virtually.
Please note: ONLY Florida residents are eligible for this clinic. If you live outside of the state of Florida, you will need to complete a legal name change in that state in the county where you currently live. If you do not need to change your legal name but you need assistance with amending your U.S. Passport to align with your gender identity, please review

Reminder: At this time, neither state agencies (FLHSMV and FDOH), nor the Social Security administration, are amending gender markers on government-issued identification documents, but anyone can still change their legal name to reflect their gender identity and have that name reflected on all documents.
