LIMITATION OF LIABILITY: I as the Participant of my own freewill and/or Parent/Guardian represent that participant, as a party to this agreement, acknowledge that there is inherent risk of injury associated with virtual classes, exercise, physical training, fitness classes, yoga classes, strength training classes and other forms of exercise. I discharge from liability and waive any legal action whatsoever against both Kassia Oaks, instructor, and the host site. Participant and Parent/ Guardian acknowledge that online classes are done in a place of participants choice and releases Kassia Oaks of liability for equipment used or space utilized to perform exercise. Participant and Parent/ Guardian further warrant and represent that participant in virtual fitness classes and, as a party to this agreement, acknowledges sole responsibility for injuries caused by participation in online classes provided by Kassia Oaks. Any and all equipment used by participants is not property of Kassia Oaks and participant will use any equipment that they so choose at their own risk. PARTICIPANT HEALTH WARRANTY: Participant and Parent/ Guardian warrants and represents that the participant to this agreement has no disability, impairment or ailment preventing him/her from engaging in active or passive exercise or that will be detrimental to his/her health, safety or physical condition if he/she does so engage or participate. Participant and Parent/ Guardian understands that Kassia Oaks is not in any way responsible for Participants actions or his/her capability for physical training. PHYSICAL LIMITATIONS OR CONDITIONS: Participant and Parent/ Guardian, agree to provide written evidence of any medical conditions, limitations or problems that may affect participants physical and/or mental well being while using Kassia Oaks' facilities. You will provide this evidence prior to participating at any given time. In absence of such evidence, it is considered by Kassia Oaks that you represent yourself to be in good health, and able to participate. BY SUBMITTING BELOW, PARENT / GUARDIAN ACKNOWLEDGES THAT HE/SHE HAS THOROUGHLY READ AND AGREES WITH THIS ENTIRE DOCUMENT INCLUDING THE LIMITATION OF LIABILITY , PARTICIPANTS HEALTH WARRANTY and PHYSICAL LIMITATIONS OR CONDITIONS contained in herein. I agree to pay Kassia Oaks via cash, check, or Venmo app all sums of money agreed for exchange of requested yoga practice/teachings. *
