Feedback form to be discussed and completed by the ship's onboard training instructor(s) and apprentice together during the last days. It will take 5-10 min to complete this form and the feedback is important for the student's further education.

NOTE! Ship's onboard training instructor(s) is the persons who has been instructing the apprentice during the onboard training.

The information will only be shared and used by the student's responsible teachers at school.
