It is free to receive a new certificate if you are completing this form within 28 days of the original certificate being dispatched.
If you are completing this form 28 days or more after we let you know that the certificate was dispatched, a recertification fee of £30.00 (including VAT) will apply. If a fee is required, we’ll send you a secure payment link after the form is received. The replacement certificate will be issued once payment has been completed.

Once the form is submitted, we’ll apply for a new certificate and document number. This will automatically invalidate the original certificate.
