You will need to submit this form to Mr. Toledo by April 1st if you want him to write you a letter of recommendation. Please have your "personal statement" complete before filling out this form.
Application Packages are submitted online, and detailed instructions will be provided to help you with the submission process. If you are unable to submit your package online, please call the Office of Access and Enrollment at 773-553-2060, weekdays between 8 am and 6 pm, for alternate submission options.
