Request for Electronic Report of Examining Committee (REC) Form
All remote defenses are automatically approved by Dean Fetter for spring and summer 2021.

Please use this form to request the Digital Report of Examining Committee, which all committee members will use to sign electronically to indicate that the student has passed their defense. Before completing this form, you will need to have the following information: Student's name, UID and email, information from the nomination of committee form including the names and emails of all committee members for this defense, as well as the dean's representative (for doctoral dissertations). *
At least six weeks prior to scheduling a thesis or dissertation defense, the committee needs to be approved by the Office of the Registrar via the Nomination of the Thesis or Dissertation Committee form. Has the committee already been approved by the Office of the Registrar? *
