Childcare Supplement Request Application

We understand that the expense of childcare can be a deterrent to participating in a Life Group. Because we are committed to removing as many obstacles as possible, the church supplements group participants at a predetermined rate for their child care expenses, if needed.

To be eligible to submit childcare supplement requests online, you must:

1.Fill out the form below

2. MAKE SURE YOUR ATTENDANCE IS RECORDED in Groups on the Planning Center (Your group leader will record attendance at each meeting.)

Note that childcare supplement requests will only be accepted for events occurring within the last 30 days. Please make your requests on a monthly basis. The supplement amount is as follows:

$10 for 1st child;

$5 per/child thereafter from same family unit per each meeting date.
