Thank you for being a part of the music program. We are happy that your student will be participating in the RUSD band & orchestra programs. If you have not rented a musical instrument from a local music store, this form will allow your student to rent an instrument through the district.

Please be sure your student communicates with their band/orchestra teacher about the availability of renting an instrument. Your student's music teacher will not be able to check out an instrument until receiving this completed contract.

Rental Fees: Instrument rental through RUSD is priced at $55.00 per semester ($110.00 per year). Following your student's instrument checkout by the teacher, this fee will be electronically invoiced and applied to your student's Infinite Campus profile.

Rental fees may only be waived for families who have successfully completed the Alternate Household Income form (and meet eligibility criteria), or have received a Direct Certification letter of eligibility. Additionally, completion of the Sharing Information with Other Programs page is required. It is important to emphasize that charges related to instrument damage or loss are not subject to waiver and remain the responsibility of the Parent/Guardian.

Precio de alquiler: El precio para alquilar un instrumento por medio del programa de música de RUSD es de $55.00 por semestre ($110 por ciclo escolar). Después de que el maestro asigne un instrumento de su estudiante, esta cuota se cobrará electrónicamente y se aplicará a su cuenta en el Portal de Padres en Infinite Campus

Solo se podrá aplicar un descuento a las cuotas de alquiler para las familias que hayan rellenado y entregado el formulario de Ingresos Alternativos del Hogar (y se compruebe que son elegibles), o que hayan recibido una carta de elegibilidad por Certificación Directa. Además, se requiere completar el formulario Compartir Información con Otros Programas. Es importante enfatizar que cualquier cargo por daño a, o pérdida del instrumento no están sujetos a ningún descuento y siguen siendo responsabilidad de los padres/tutor.

I agree to the following terms, recognizing that my student will undertake the responsibility of using a school-owned instrument.
