This Membership Agreement (“Agreement”) is entered into by and between [Business Name] (“Provider”) and the undersigned member (“Member”).

Term of Membership
Member agrees to a one (1) year membership commitment, beginning on the date of enrollment (“Start Date”) and continuing for twelve (12) months (“Initial Term”).

Automatic Renewal
Upon expiration of the Initial Term, this Agreement will automatically renew for successive one (1) year terms (“Renewal Term”) unless Member provides written notice of cancellation at least thirty (30) days prior to the renewal date.

Payment Obligation
Member agrees to pay the full membership fee for each Term (Initial and Renewal), regardless of actual usage. Payments may be collected monthly, quarterly, or annually, as specified at the time of enrollment.

Cancellation Policy
If Member cancels membership before the end of the current Term, the full remaining balance of that Term will become immediately due and payable at the time of cancellation.

Changes to Terms
Provider reserves the right to modify membership pricing or terms upon renewal. Members will receive notice of any changes at least thirty (30) days prior to renewal.

Acknowledgment
By signing below, Member acknowledges that they have read, understood, and agree to the terms outlined above, including the automatic renewal and payment obligations.
