DIVER WAIVER
In consideration of my (and/or my minor child/ren) participating in activities or otherwise sponsored by or on behalf of Saint George Greek Orthodox Church of  Ocean City, Maryland (the “Church”) (the “Activities”), I (which for purposes of this Release and Waiver, includes on behalf of myself and any minor children indicated below, and the heirs, assigns and executors of each of us), agree to this Release and Indemnity Agreement (“Agreement”) intending to be legally bound: 

RELEASE. I do hereby fully and forever release, waive and discharge the Church and its past, current, and future officers, directors, clergy, employees, members, volunteers, contractors, representatives, parents, parish council members, affiliates, diocese, archdiocese, and agents, successors, and assigns of each of the foregoing, (collectively, “Activity Provider”) from any and all damages, injuries, losses, liability, claims, causes of action, litigation, or demands, including but not limited to those for personal injury, sickness, or death, as well as property damages and expenses, of any nature whatsoever which may be incurred, directly or indirectly, in connection with participation in the Activities or any travel related thereto (“Claims and Damages”). In addition, I promise not to sue the Activity Provider related to any of the foregoing. 

It is expressly understood and agreed that this release is intended to cover and does cover not only all known injuries, damages and losses, but any future injuries, damages and losses not now known or anticipated but which may later be discovered, including all effects and consequences. It is understood that this is a complete release and that there are no written or oral understandings or agreements directly or indirectly connected with this release that are not incorporated herein. 

INDEMNITY. I hereby agree to defend, indemnify and hold harmless the Church and the other Activity Providers from any and all Claims and Damages, and/or Inherent Risks. 

If Participants Include One or More Minors: In addition, the undersigned hereby represents that he or she is: (i) the parent or legal guardian of the minor participant(s); (ii) authorized to, and does, enter into this Agreement on behalf of all parents and legal guardians of the minor participant(s); and (iii) is executing this Agreement individually and on behalf of minor participant(s).
