Each player must complete the Release & Consent form for the 2023 year before participating in any Thunderbird Event.

MEDIA RELEASE: As the parent or legal guardian of the player listed below, I grant Thunderbird Golf my permission to use any/all photographs taken during Thunderbird events for any use, including but not limited to: publicity, copyright purpose illustrations, advertising, marketing content, etc.

LIABILITY RELEASE: As the parent or legal guardian of the player listed below, I release Thunderbird Golf from any responsibility regarding any personal injury claims for the child mentioned above.
