The Nashville LGBT Chamber presents "Take PRIDE in your Health!" - a first-ever, inclusive, LGBT-friendly healthcare provider expo.

Please complete this application for consideration. Space is limited to 50 booths. We would like diversity in representation based on type of service you provide. Priority will be given to LGBT Chamber members.

What you Get
● Access to LGBT Chamber members and non-members who attend; we anticipate 150-200 people the first year. This event is FREE for all attendees, regardless of LGBT Chamber membership status.
● One 6-foot table
● Chairs (up to 2), upon request

What you Need to Bring
● Any promotional items you wish to distribute.
● Decor/signage for your table.
● Adequate staffing. Your table must be staffed at all times during the event.

What you Need to Know
● Set up begins at 3:00pm and ends at 4:00pm.
● Load-in may be done via the 19th Ave. parking lot.
● Tear down and load out will begin at 7:00pm, and you are to have all of your belongings moved out of the venue by 8:30pm. Vendors may not tear down before 7:00pm.

Cost to exhibit
● Nashville LGBT Chamber Members are FREE (with option to upgrade annual membership at 20% off)
● Non-members are $300 with one-year Copper membership (with option to upgrade annual membership for 20% off)

Rules and Regulations
The following terms and conditions shall apply to this agreement and are binding upon the parties hereto:

1. Space will be assigned upon completion and return of this agreement along with the Certificate of Insurance.
2. Vendors will use only the space assigned to them. Registration includes one 6-foot table and two chairs
3. Vendors will enter the University School of Nashville at 2000 Edgehill Avenue, Nashville, TN 37212 and volunteers will show them to their location.
4. Set up time is 3:00pm to 4:00pm. Load-in may be done via the 19th Ave. parking lot. Vendor must be prepared to exhibit when the health expo starts.
5. Vendors requiring electricity will need to provide their own extension cords and power strips. All attempts will be made to accommodate those needing access to electricity. However, Vendors should be prepared if electrical access is not possible.
6. All vehicles must be removed from the load in area immediately after unloading.
7. Vendor booths are to remain intact until the closing at 7:00pm. All booths must be dismantled and items removed by 8:30pm.
8. The Nashville LGBT Chamber of Commerce reserves the right to decline to permit a vendor to conduct and maintain a booth if, in the Nashville LGBT Chamber’s sole judgement, said vendor shall, in any respect, be deemed unsuitable. This reservation relates to persons, conduct, articles of merchandise, printed matter, souvenirs, catalogs, and any other thing without limitation which affects the character of the booth and, therefore, this event.
9. It is specifically declared that all vendors will confine their activities to conform to specifications set out by the venue’s rules and regulations, and vendors shall abide by the general agreements between the Nashville LGBT Chamber and the venue, and also for specifications of the Metro Police Department and Fire Marshall’s Office for the premises.
10. Vendor agrees to protect, save, and hold the Nashville LGBT Chamber of Commerce and all members and officers thereof forever harmless for any damages or charges imposed for violation of any law or ordinance whether occasioned by the negligence of the vendor or those holding under the vendor, and save and hold harmless the indemnities against and from any and all losses, costs, damage, from or out of by reason of said vendor’s occupancy and use of the premises or any part thereof. It is recommended that vendors take individual precautionary measures such as securing easily transportable articles of value and their removal to a place of safekeeping before, during, and after event hours or while the booth is not manned. It is further agreed that all material brought by the vendor is the responsibility of the vendor, and the Nashville LGBT Chamber of Commerce hereby disclaims all responsibility of these articles. The parties hereto acknowledge that the foregoing disclaimer of liability has been negotiated between the parties and is reflected in the shares applicable to this agreement.
