TENNESSEN WARNING:
The Shared Services Alliance (SSA) program, funded by the
Department of Human Services (DHS), supports family child care providers in
sharing services to reduce costs and achieve efficiencies within their program.
Our SSA staff will need the following information to best serve you: [Name,
Email, License, Location, Provider Demographics, etc.]. Once you are a member
of SSA, additional information may be collected related to your child care
program, including items such as: program demographics, program capacity and
enrollment, Parent Aware participation, income, business management practices,
business expenses, and services received through the SSA. Any information you
share with Milestones will be governed by the Minnesota Government Data
Practices Act.
Why must we collect this data?

coordinate the requested contact from a member of our shared services alliance
team
· To
determine eligibility for services or benefits from the shared services
alliance program
· To
meet reporting requirements, evaluate the shared services alliance program and
evaluate the cumulative impact of services offered by the State.
Do you need to give us this information?

No, participation in the shared services
alliance is voluntary
·
However, if you choose not to provide the
requested information, you may not be eligible for services available through
our shared services alliance. For
example, our shared services alliance team may not have information to contact
you to answer your questions
Who will have access to my information?
· We
will only share your information as authorized or required by law.
· The
shared services alliance program being offered is funded by DHS through third
parties. Milestones may share my information with DHS, with their contractors
who need this information to do their jobs, with researchers conducting
evaluations of the shared services alliance program and related programs, and for
limited exceptions authorized by law.
