2024-25 GVMP Object Collection Form
Thank you for your willingness to contribute an object for the Gun Violence Memorial Project. The following questions relate to you, the person you are honoring and the object you want to contribute. Please take a moment to complete the form ahead of coming to drop off your object.

The memorial will be on display at the Museum of Contemporary Art Detroit from May 2nd - August 10th. For more information on the exhibition, please visit the website here.
