Concord Public Schools and Concord-Carlisle Regional School District are committed to taking the necessary steps to confront and prevent incidences of bias incidents and hate crimes in our community. We appreciate your assistance in bringing these incidents to our attention. If you believe you may have been a victim, witness, or received third-party information about a bias/hate-related incident or hate crime, click here read specific steps to address the incident. All information leading to a formal submission of a reporting form, will be collected in order to investigate allegations of bias or hate crime. All information relating to a hate crime, is subject to federal and state criminal investigation protocols.
1) This Reporting Form can be completed by the victim, a witness, or a third person (for example, a parent or teacher). The reporter can remain anonymous but try to include as much information as possible.
2) The school Principal or their designee will receive the incident report, acknowledge the report within one school day, assess the needs of the incident, and determine the next steps for an investigation. All incident reports are confidential to the extent possible unless it requires judicial action.
3) Report immediately any bias incident(s) related to vandalism, physical assault, property damage, or bullying, harassment, or sexual assault to a trusted adult (school Principal, teacher, parent/guardian or Concord Police).

Confidentiality Statement
Concord Public Schools and Concord-Carlisle Regional School District are dedicated to safeguarding the privacy of all individuals involved in a report of a bias-motivated incident or hate-motivated crime. Every endeavor will be made to ensure the confidentiality of the individuals targeted and to uphold the confidentiality preferences expressed by the reporting party. The resolution of any reported incident will take into account the confidentiality of all parties involved.
