Birthday Lunch Form
Welcome to our Birthday Lunch program for 2025-26!  This year we are offering plain pizza from Pizza Brothers and desserts from Catering Solutions.  You can order pizza, dessert or both to celebrate your child's birthday! 

STEP 1:  
Complete and submit this form so our Cafeteria Volunteers and Office Staff are informed about your delivery.  This form is required at least 5 days in advance of your child's celebration.

STEP 2:
If ordering pizza, complete the CheddarUp form (link contained in this form) to purchase the pizza.  The HSA will place your order with Pizza Brothers.  You DO NOT need to place the order with Pizza Brothers.  

STEP 3: 
If you are ordering dessert, please contact Catering Solutions at 908-566-6414 or place your order directly through their ordering website.  Look for snack options and cupcakes in the add-on section of their ordering website.  

STEP 4: 
Notify your class via text or email about your celebration so parents can make necessary changes to any existing lunch orders.

If you have fulfilled all volunteer requirements and will be joining us in the cafeteria to help serve your child's birthday lunch, please enter through the front office to obtain a Visitor's Pass. Volunteers may not enter directly through the Cafeteria entrance.

Note: Younger unenrolled children are not permitted to attend.

Please note, this is not an approval form. This form is used to alert Cafeteria Staff to potential staffing needs, Front Office staff to potential visitors, and to instruct the HSA to place your pizza order. For questions about the Birthday Lunch Program, please email hsainfo@steschool.org.
