Calvary Community Care - 2025 REGISTRATION FORM

Dear Parents,

Calvary Community Care (C3) provides a range of literacy support programmes to help children develop reading proficiency.

Please fill up this form to register your child for the programme/s according to his/her needs.

The sessions follow the MOE school semesters.

LESSONS WILL START FROM FEBRUARY 2025 (first intake for the year)

Online one-to-one GROW lessons are once a week - $30 per lesson (a total of 28 lessons)
(Limited onsite lessons are available, subject to a minimum number to start.)

Online iREAD sessions are twice a week - Free
Semester 1：Feb-May 2025
Semester 2：Jul-Oct 2025

Onsite and online StoryLAB sessions are once a week - Free (a total of 28 sessions, Feb-Nov)
(Online StoryLAB sessions only available for referrals)

Subsidies are available if required. (Documentation will be needed.)

PDPA CONSENT

Calvary Community Care (C3) collects personal information like names, contact details, photos, videos etc during the course of this Programme. 

I fully understand and agree that the personal information which I have provided or is collected during C3 activities may be used or disclosed for the purposes of:
(a) Engaging me/my child regarding C3 programmes, services and activities;
(b) Record keeping, storage, audit and promotion of C3 and her programmes;
(c) Inclusion in publicity videos, brochures and other collaterals. Information collected may include photographs, audio recording, videos or other media.
(d) Updating partners and sponsors regarding the progress of the children.

I agree for C3 to contact me for purposes relating to C3 programmes, services and activities and maintaining the ongoing relationship with C3 until/unless I state otherwise.

INDEMNITY

I confirm that my child’s participation in this Programme and the related activities is entirely voluntary and I accept all risks involved therein. Accordingly, I hereby indemnify and shall not hold liable Calvary Community Care (C3) and/or any of their employees, volunteers or partners for any loss, damage (including indirect or consequential loss and/or damage), injury or illness suffered by me or my child as a result, directly or indirectly, of participating in the programme.

I understand that my child's GROW / iREAD teacher may need to contact me directly regarding the lessons and I agree for my contact number to be shared with the teacher who is teaching my child.  *
I understand that there will be no replacement lessons if my child is absent. *
I understand that it is important for my child to attend the lessons and that C3 will terminate the lessons for my child if my child is absent for 3 consecutive lessons without a valid reason or is absent for more than 50% of the lessons in a semester. *
I understand that the fees paid are non-refundable should I withdraw my child or should the lessons be terminated by C3 due to my child's poor attendance. *
