VOE Request Form
The Texas DPS office requires VOEs for all first time applicants of drivers permits. Please complete this request form and give at least 24 hours before pick up in the front office. Forms are only good for 30 days, so only request within 30 days of your appointment. 

If a student currently owes any  attendance hours they will not be able to receive their VOE until all the hours have been made up.
