I understand that this tournament could possibly go over the scheduled end time. Whether or not that is the case, I am still expected, as a member of this team, to be present at the tournament from start to finish. I understand that if I arrive after the scheduled check in time or I leave the tournament before I am dismissed by the head coach I will be marked as absent from the tournament for the entire day and will be required to complete a disciplinary task before I can attend another tournament.

I understand that I am responsible for my judge and that if my judge agreed to attend this tournament, does end up being scheduled for it, and does not arrive to fulfill their necessary duties, on time or at all, I or my judge will be charged the missing judge fee.

I understand that once I submit this form, that is my status regarding competing at this tournament and that is what will be marked in the coach's records. This is the reference that the coach will use when registering the team. If anything changes after I submit this form I must notify the head coach right away so he can make the proper changes to the registration. I understand that if I marked myself as attending this tournament and I end up needing to be dropped after the registration deadline, I am responsible for paying the drop fee that comes with that change.
