EHS Attendance Form

Here is the attendance form. Please complete this when your student is arriving late, being dismissed early, leaving for an appointment, or out for the day.

Important: If your student is leaving early or coming in late - please have them stop by the main office for a blue slip. A blue slip allows 1) A teacher to know the student can leave the class, 2) Prevents the Main Office staff from calling and interrupting the class to get your student out, 3) You to stay in your car and wait.

If your student is 18 or older, you can complete the Age of Majority form, located in the main office. This form allows your student to sign themselves in and out of school for a valid reason only.
