I grant permission for my child/youth's image(s) to be posted in First Unitarian Universalist Church of Winnipeg media including but not limited to the website, newsletter, social media platforms, videos, etc. Children/youth will not be named.
My child/youth may participate in supervised activities outside: on the church grounds, going for a walk, etc. Please note: we are on the bank of the Assiniboine River and do not have a fence. When children are participating in Religious Explorations on Sunday mornings they are supervised. At all other times, children are the responsibility of their parent/guardian.
Occasionally children/youth may have events off site, and rides may be arranged. You will always be aware of these ahead of time. Do you give permission for your child/ward to ride in vehicles (always using a seatbelt) to and/or from offsite Religious Exploration Program events, as planned?
We want to provide the most positive experience for your child. Please describe anything that would be helpful for use to know - strengths, areas for growth, behaviors, accessibility needs, health, learning needs, family situations, etc. The information you provide will be shared with your child's RE Team. If you would like to discuss anything additional with the DLFD, or something changes throughout the year, please be in touch with her. She is available to support your family with pastoral care and/or simply pass along information to volunteer teams.
I will monitor my child for safety and behaviour before and after RE Program hours; this includes in the Sanctuary, in the House, and on the surrounding property. Please note: our chalice, our piano, and some of our people are breakable. We talk to children about walking while in the church and being careful for their own safety during coffee hour. We hope you'll echo this message.
I release the First Unitarian Universalist Church of Winnipeg, its Board of Directors, staff, and volunteers from all liability with respect to my child/ward's travel to and participation in any planned events. This authorization covers the church year from July 1, 2019 to June 30, 2020
