2018-19 IKES PTA Membership Application
Please complete the following information. When you have completed all sections of the application, please click on the SUBMIT button and then submit payment to IKES PTA. A copy of your application will be emailed to the email address you provide for your records.

*METHODS OF PAYMENT*

1. Pay In Person: Cash, Checks, and Credit Cards are acceptable forms of payment for PTA Membership at Back To School PTA Events.

2. Send Payment into School with Student: You can also send in payment for PTA Membership to school with your student in the form of cash or check (made out to IKES PTA). Please send payment in an envelope clearly marked "IKES PTA" and include the following information with your payment: (a) student's name, (b) student's grade, (c) teacher's name, (d) contact person's name, (e) contact person's email and (f) contact person's phone number.
