Transportation Custody Stop Change

This form is to be completed ONLY for students with a split custody situation. Proper documentation will be required when requesting split custody stops.

All custody stops must be submitted to the transportation department prior to July 15th every school year. If the form is not submitted prior to July 15th, it will delay the changes being made for your student and can take up to TWO WEEKS AFTER SCHOOL STARTS. 

The Plum school district can provide transportation service for a bus stop location as long as the stop is WITHIN THE ATTENDANCE ZONE of the school in question. These requests depend upon the availability of bus service, bus load limits, and time constraints. Requests are handled on an individual basis.
