New Client Intake Sheet
Thank you for your interest in retaining me to assist you with your legal matter. I charge $25 for a 30 minute initial consultation. ONCE YOU SUBMIT THIS ONLINE INTAKE SHEET, you will be prompted to CLICK A LINK TO VIEW MY CALENDAR AND SCHEDULE your initial consultation. YOU WILL ALSO RECEIVE AN EMAIL to the email address provided confirming your submission of this form and a link to schedule your initial consultation.
IT IS YOUR RESPONSIBILITY TO SCHEDULE YOUR INITIAL CONSULTATION. If you do not request an initial consultation using the links provided, you agree that I may contact you by any one of your authorized means to assist you with the process. However, your authorization does not create a duty for me to contact you.
WE ARE NOT ENTERING INTO AN ATTORNEY CLIENT RELATIONSHIP BY YOU COMPLETING THIS INTAKE SHEET. DOCUMENTS SUBMITTED PRIOR TO ACCEPTANCE OF AN INITIAL CONSULTATION REQUEST MAY NOT BE PROTECTED BY THE ATTORNEY CLIENT PRIVILEGE. It is recommended that you bring with you or deliver a copy of any and all documents relating to your legal issue for review at the time of the initial consultation.
I WILL NOT DO ANY WORK ON YOUR BEHALF UNTIL SUCH TIME AS WE BOTH HAVE SIGNED A WRITTEN RETAINER AGREEMENT. However, in exchange for pre-payment of $25, I will provide you with a 30 minute initial consultation after you have cleared an internal conflict of interest check based on the information you have provided. If I am unable to provide you with an initial consultation due to a conflict of interest, I will decline your appointment request and you will not be charged an initial consultation fee. If I decline your appointment request, you should seek other counsel.
