MSL 2021 Participant Opt-in form
The purpose of this ‘Opt-in’ form is to invite you, in your capacity as a Participant (player / coach / umpire / scorer / accompanying person) to return to sport or physical activity in the context of the coronavirus (Covid-19) pandemic (“Return to play”); and to set out the terms and conditions which you must comply with if you wish to return to play.

Personal information collected from this form is subject to Data Protection laws, and will not be used for any purposes other than Track and Trace and for player registration with the BSF for insurance purposes. In particular, your email address will not be used by MSL for any other purpose, and will not be passed on to any other person or organization.

Please note that you only need to complete this form once for the whole MSL league season. Tournaments (including any organized by the MSL) will require a separate opt-in.

Please note that you are not required to return to play, and if you choose not to return to play then you will not face discrimination by us. If you do ‘opt in’ and subsequently change your mind, you may withdraw your consent at any time by contacting the MSL-COVID-19 Officer
I, the Participant, hereby confirm and agree that I wish to return to play; and in doing so I confirm that:
(i) I do not have, and nobody in my household has (including within the past 14 days), any of the symptoms of coronavirus, as set out on the NHS website1 from time to time, including without limitation a high temperature, a new continuous cough, or a loss or change to sense of smell or taste;
(ii) I will immediately inform the Activity Provider if I (or anybody with whom I have been in close contact) develop coronavirus symptoms. In such circumstances, I shall follow the Government’s guidance in relation to self-isolating and I shall not attend the Activity Provider’s event, premises or facilities;
(iii) I will comply with all relevant guidance, rules, policies and advice published or made available from time to time by the Government, the World Health Organisation and the Activity Provider in relation to coronavirus, including without limitation in relation to applicable social distancing measures;
(iv) I understand and accept the potential risks associated with returning to play in the context of the coronavirus pandemic, including without limitation the risk of unintended transmission of the disease, and I will comply with all of the Activity Provider’s reasonable instructions in connection with coronavirus;
(v) If required by the Activity Provider from time to time, I agree to complete a medical declaration form prior to being permitted entry to the Activity Provider’s premises or facilities. I acknowledge that any failure to do so may result in access being denied; and I give my consent to my personal data (including any ‘special category personal data’, such as data pertaining to my health) being processed by or on behalf of the Activity Provider, in accordance with the terms of its Privacy Policy* from time to time. Further, I consent to my personal data being used for the purposes of health and safety, including without limitation in connection with the Government’s coronavirus Test and Trace2 policy.
