Parent District Clearance Request

Thank you for wanting to help the Brigade, we really could not do anything without you!! Filling out this form will start the process to become a cleared chaperone or trailer driver for the Emerald Brigade at Granite Bay High School. Clearance is required to chaperone ANY trip, ride the bus with the students, drive any students, or tow any of the trailers. There is an additional and separate clearance process to be able to drive the students.

Once you fill out this form, Mrs. Lewis will submit it to the district and they will send you a packet. The packet prompt you to submit:

District forms

Once you turn in the packet to the district, they will notify you as well as Mrs. Lewis that you are cleared to chaperone students or tow a trailer. If you are planning on driving students, you need to also be cleared by the district's transportation office which is a separate process.
