Service Learning Daily Log and Time Record
In order to receive credit for service learning and be eligible for transcript notation, you must log your service. This form will serve as your service learning timesheet for official documentation of your daily service learning activity. That is, all time you spend at or online with the service site, researching, gathering information, meeting with fellow students or agency representatives, and executing aspects of your service learning. Daily log the start and end of each service assignment. Please note that (1) reporting of hours is subject to Loyola’s policies about academic integrity; and (2) intentionally falsifying hours is a form of cheating.  Your hours and progress will be monitored and reviewed regularly.
