Please complete for baptism of children through the age of 12.  This request includes information regarding the child and parents.  Immediately following this information, you will be asked to complete a Baptism Bible Study.  Upon submission, your Christ Church Baptism Liaison will review your request with the pastoral staff and contact you to confirm the requested date.
