2017-18 Middle School Registration Form
Middle schools must be registered and payment of dues must be postmarked by October 28, 2017.

This form must be submitted online. After the registration form is submitted, an invoice will be mailed to you immediately.

Payment must be snail mailed to the IHSFA.

Registration is only considered complete upon receipt of payment.

2017-18 ANNUAL DUES:
Payment should be postmarked by October 28.

Submission of this form certifies that the named coaches are responsible for the entry of the school in middle school tournaments. They are certified to enter your school's students for middle school tournaments. All entries submitted must be full time students in good standing at their middle school. A VALID AND PROPERLY TYPED EMAIL ADDRESS IS REQUIRED FOR THIS FORM.
