To help you reflect on your internship experience and learning, please consider the following questions and provide a self-assessed rating for each item.

Reflecting on your skills and goals will increase your self-awareness and growth, and this form will provide your Faculty Supervisor with an update so they can determine what guidance and support will be most helpful throughout the rest of your internship.

You will receive an emailed copy of this evaluation form after you submit it, at the email address you provide. Once you receive it, forward it to your Faculty Supervisor (a requirement for your grade) and On-Site Supervisor (so you can have a conversation about your performance and progress in your internship).
