I, the undersigned, hereby acknowledge that I have voluntarily chosen to participate in the Broadway Fire Department Golf Tournament. I understand that participation in this event may involve inherent risks, including, but not limited to, injury, loss, or damage to personal property.

In consideration of being allowed to participate in this event, I hereby release, waive, discharge, and hold harmless the Broadway Fire Department, its officers, employees, volunteers, and sponsors from any and all claims, demands, or causes of action that I may have for any injury, loss, or damage arising out of my participation in this tournament.

I further acknowledge that I am responsible for my own safety and well-being during the event and will adhere to all rules and regulations set forth by the organizers.
