Parent Form: Social-Emotional Learning
Thank you for your time in helping your child's school counselor better understand the social-emotional needs of your child. In order to focus on learning, we first need to make sure student's social, emotional, and behavioral needs are met - and YOU know your child best. 

Directions:
Please read the descriptions listed under each of the five sections below. Rate your child based on the way you see them acting at home. While all questions are optional, we highly encourage you answer each.
