Thank you for your interest in attending the Community Health Worker (CHW) Network to be held on Wednesday, April 21, 2021 from 12:30 pm-2:00 pm online via Zoom. Please fill out the form below so we can send you meeting details, Zoom invitation and reminders.

The meeting will be an opportunity to build your skills, network with others working as Community Health Workers or in similar fields, and build momentum for Community Health Workers throughout Sonoma County.
