9th Grade Study Trip
The dates for the Freshman Study trip are March 1-2, 2018. Destination is Monterey area. Additional details to follow.
Student seats to be covered: 108. Estimated # of drivers needed: 26 (exact amount depends on vehicle type of drivers)

1) Driver Form
2) Chaperone Agreement
3) Original or a high-quality copy of a negative TB test - is no more than 60 days old from the first day of volunteering. Valid for 4 years.
4) Fingerprinting Clearance. Please print and use the SPS Request for Livescan (on parent portal). This is the form that you must use to complete your Livescan. It contains a special code that will ensure your results are sent only to Summit. You only have to do this once.
5). Driver license
6). Registration (of vehicle you plan on driving on trip)
7). Insurance (of vehicle you plan on driving on trip) with the following minimums:

$50,000 for personal injury to, or death of, one person,
$100,000 for personal injury to two or more persons in one accident
$50,000 for property damage.
