Senior Connection Center Mini-Grant Application Criteria - 2018

The mission of the Senior Connection Center is simply to help older adults and persons with disabilities live with independence and dignity. This is a powerful statement and a noble cause that we strive to reach on a daily basis through our varied programs and services. As our name implies, we connect older adults, caregivers and persons with disabilities to the resources they need, as well as provide critical information where they can make more informed decisions about the resources they are seeking.

We are the Area Agency on Aging and the Aging and Disability Resource Center for the counties of Hillsborough, Manatee, Polk, Highlands and Hardee. We are charged with developing a comprehensive and coordinated system of services throughout our communities that is focused on meeting the identified needs of those we serve. It is imperative that we stay connected to our communities through developing public/private partnerships, conducting needs assessments and outreach events, as well as receiving guidance from a very engaged Board of Directors and Advisory Council.
The vision of the Senior Connection Center is to be the recognized leader in our community who connects older adults, persons with disabilities and caregivers with the information, resources and services they need. This vision is linked clearly to our mission because if individuals receive the services they need, they will live independently and with dignity.

Senior Connection Center’s Board of Directors is committed to improving the lives of older adults and persons with disabilities. As a way of accomplishing this, SCC’s Board will provide a limited number of mini-grants on an annual basis to address unmet needs impacting vulnerable seniors and persons with disabilities residing in Hillsborough, Manatee, Polk, Highlands and/or Hardee Counties.
