CREW Nights Summer 2020 | Parent Consent
This summer we are so excited to be launching CREW Nights beginning on Wednesday, July 15th! The CREW is our student serving team at Uprising and it is made up of students that have chosen to take the next step in their faith by using their gifts to be a part of what God is doing in our community. For these students, Uprising isn't just something they attend - it's something they get to be a part of!

Like Uprising in the Wild, CREW Nights will happen every second Wednesday and they will be open for all students currently serving on the CREW or those that are interested in joining! During these evenings, students will have the opportunity to dive deeper in their faith, engage in meaningful community, and discover more about the purpose God has for their lives through different fun group activities, leadership training, discussions, and more! If you are not currently serving on the CREW but would like to, go to for more info and to sign-up!

Take a look below for more information about the different CREW Nights happening this summer and the steps we are taking to keep your students healthy and safe! Then please fill out this short form to indicate that you have read your responsibilities as a parent/guardian and understand the steps that Uprising will be taking to provide a safe space for your child. All students participating in CREW Nights must have submitted a consent form before participating. You will only need to fill out this form once this summer.

Time - 7:00 - 9:00 PM
Drop-off - Half Moon Drive-In (6860 Henderson Highway, Gonor, MB)
Pick-up - Duff Roblin Parkway Trail (off the 202 highway before Half Moon Drive-In -- directions to be provided via email)
Activity - Ice Cream & Hiking

Time - 7:00 - 9:00 PM
Drop-off & Pick-up - The Home Depot (1999 Bishop Grandin Blvd)
Activity - Biking to St. Vital Park and back.

Time - 6:00 - 9:00 PM
Drop-off & Pick-up - Birds Hill Park (Group Use Area #2, Site #5)
Activity - Bonfire & Beach
***Students will need to bring their bikes.

Time - 7:00 - 9:00 PM
Drop-off & Pick-up - The Forks (in front of the Children's Museum)
Activity - City Crawl & Picnic

*More details will be provided by email.
**As of now, all students will be required to be dropped-off and picked-up at each location. We are currently looking into the possibility of alternative transportation should it be required and more information about that will be provided soon.
I/we hereby give permission for my/our child to participate in Uprising Middle School summer gatherings.

I understand that it is my responsibility to:
- Remind my child to practice safe social distancing while at any Uprising gathering
- Choose what preventative measures my child will take (i.e. mask, gloves, etc)
- Ensure that my child does not participate with any symptoms of COVID-19
- Pick up my child should they not be following social distancing protocols
- Drop-off and pick-up my child at the specified meeting point, unless other arrangements have been made.

I understand that it is Uprising’s responsibility to:
- Provide hand sanitizer for students to use upon arrival and departure
- To keep gatherings in an outdoor space with minimal indoor activity (students may use the washroom)
- To refrain from organizing any activities that do not allow for social distancing
- To limit and avoid activities that involve touching objects and cross-contamination. In the case where activities require touch, steps will be taken to ensure the regular sanitization of all objects.
- To take extra precautions to clean all frequented spaces, including doors and bathrooms.
- To contact parents if their student refuses to follow the social distancing protocols and ask that they be picked up.
